Both directions, one answer

Open a person and see what they hold. Open an item and see who has it and who had it before. The same record read two ways, so the two can never disagree.

HR and admin assign it

Issuing an asset is an action in the system with a date and a name against it, rather than a note in a drawer.

A handover letter, on your letterhead

Generated from the assignment itself — the item, its tag, its condition and the date — so the paper and the register say the same thing.

Offboarding that checks

An exit cannot be completed quietly while four items are still out. The pending list is part of the checklist, not a thing somebody remembers.
